The Facility
Imboden Creek Gardens is located on a beautiful campus in Decatur, IL. Our campus includes Villas for an independent living option as well as a skilled nursing facility. Regardless of your level of care need, this campus can offer it all. Imboden Creek Gardens offers the opportunity to live more independently, on your terms. Life at Our Community
From the moment your loved one wakes up in the morning, our nurses, therapists, and trained staff will be there to help them work and heal throughout the day.
Their day will be filled with three prepared meals cooked by our dietary staff. We encourage you to stop in and try the food! We are very proud of our excellent dining services. Our activities department is always up to something fun. From playing games to spending time outdoors to preparing for upcoming holidays, the staff is constantly working to provide socialization and fun for our residents. Our therapy department will work with your loved one’s physician or surgeon to determine a schedule and goals for these services. Many civic and religious organizations schedule time at our communities to entertain and worship and we are ever so thankful for the communities that give so much to our residents.

Acorn Estates is a Supportive Living Community designed as an alternative to traditional nursing & assisted living. Persons with a low-income, limited financial means and or disabilities are eligible for a supportive living community. By combining apartment-style housing with personal care, Acorn residents can live independantly with dignity and privacy as well as take part in decision making.

We are proud to work with excellent hospice services in Southern Illinois. Our social services staff, nurses and administrators will work with you and your family to arrange for this care.
REHAB & THERAPY
Our team of occupational, speech, and physical therapists will work with you and your physician to determine the best plan to reach your goals in the safest means possible. Our therapists are above and beyond the best and we are proud of the excellent care they provide.
SOCIAL SERVICES
We work to provide care plans for each individual and provide excellent communication between our staff, the resident, and their family. Our social services staff are happy to answer any questions you may have.
PHARMACY SERVICES
